By participating in the survey, you will help us continue to improve our services and facilities, and ensure we are delivering the right programs and support for our members and community. The survey will take around 10 minutes to complete.

Confidentiality 

This survey is being conducted by Swinburne University, and all responses will be treated in strictest confidence. 

Prize draw 

As an incentive, all completed surveys go into the draw to win a $100 Rebel Sport Voucher. There are three prizes in total to be won. To be in the draw to win, you will need to provide preferred contact details at the end of the survey and the winner will be notified by email.
